RESUMO

RATIONALE, AIMS AND OBJECTIVES: The prenatal education promotes the empowerment of parents during pregnancy and postnatal period. This study aimed to assess the quality of educational sessions held in the third trimester of pregnancy as part of the parenting education programme for Spanish National Health System in Madrid. METHODS: The design is a cross-sectional study in 41 primary care centres in the autonomous community of Madrid, which is one of the 17 autonomous communities that constitute the Spanish State, each wick medical responsibilities. The participants are a representative probability sample of 928 attendees to the programme. The assessment instrument is 'EDUMA2' questionnaire (Cronbach's alpha = 0.829) of 56 variables. Descriptive statistical analysis was performed using SPSS. The project was approved by the Research and Ethics Committees of the University Hospital of La Paz. RESULTS: The uptake efficiency immigration risk is 14.7%, and lack of social support is 8.7%. The functionality in organization, teaching and methodology is high in 90.5%. The learning effectiveness of health habits, care and techniques is significant and greater than 60% in the 14 parameters studied. Satisfaction is very high at 67.5%. The immediate impact in terms of control or safety increase is significant and greater than 71% and significantly greater than 40% and for increasing the bonding with the baby. CONCLUSIONS: No jobs found with which to compare. The assessment of the programme with adequate psychometric characteristics questionnaire allows designing strategies and research to improve the quality of prenatal education.

INTRODUCCIÓN: Los programas de educación maternal constituyen un servicio sanitario de la sanidad pública en España para disminuir la morbimortalidad maternoinfantil. OBJETIVO: Describir los programas de educación maternal que se imparten en los centros de atención primaria de la Comunidad de Madrid y recoger las características de la población que asiste. MÉTODO: Estudio descriptivo. Muestra probabilística de 94 matronas (54,6% de las matronas de primaria de Madrid). Cuestionario de elaboración propia, validado. Análisis descriptivo. Programa SPSS. RESULTADOS: En relación con un estudio de 1997, las sesiones del primer y segundo trimestre de gestación han disminuido un 26,5%. Se mantienen las del tercer trimestre. Las de puerperio han aumentado un 22,5%. Mayor asistencia de parejas (48,5%). Las matronas reciben en los grupos a mujeres con más de tres niveles culturales diferentes en el 66,2%, y a mujeres inmigrantes de dos o más procedencias diferentes en el 77,5%. La media de asistentes por grupo es de 30. DISCUSIÓN: Los resultados permiten conocer la evolución de los programas en el sistema sanitario público de Madrid en los últimos 10 años y las características actuales de sus asistentes. Destaca el esfuerzo de las matronas para captar a las gestantes, a sus parejas y a las embarazadas inmigrantes, así como su trabajo para adecuar los programas a las necesidades propias de su población de referencia. También se evidencian las necesidades de mejora, lo que permite diseñar estrategias para aumentar la calidad


INTRODUCTION: Maternal education programs are one of the services provided by the Spanish public health service to reduce maternal and child morbidity. OBJECTIVE: Show the maternal education programs in primary health care settings in Madrid and collected the characteristics of the population who attended them. METHODS: Descriptive study. Probabilistic sample of 94 midwives (54.6% of the midwives in primary care in Madrid). Self-completion questionnaire, validated. Descriptive analysis SPSS program. RESULTS: In comparison with a study carried out in 1997, the number of sessions in the first and second trimester of pregnancy has decreased by 26.5%. The number of sessions has been maintained in the third trimes ter. Post natal sessions increased by 22.5%. Improved attendance by partners (48.5%). In 66.2% of the sessions the midwives had women from more than three different educational levels, and in 77.5% of the sessions immigrant women from two or more different areas of origin attended. Group size was on average 30. DISCUSSION: The results show the way these programs have evolved in the public health system in Madrid over the last 10 years and thhe characteristics of the women who currently attend sessions. One of the most notable features was the effort made by the midwives to attract preg-nant women, their partners and pregnant women of immigrant origin to the sessions, and to make the sessions relevant to the needs of those at tending them. There was also evidence of the need to design strategies to increase the quality

Justificación: Los programas de educación prenatal son una poderosa herramienta para aumentar la salud materno-infantil. Hoy no se conoce el enfoque educativo más eficaz. Objetivo principal: Conocer las diferencias de enfoque y metodología de las principales escuelas de educación maternal. Metodología: Revisión de las bases de datos PubMed, Embase, Lilacs, Scielo y Cochrane desde el año 2000. Revisión de las páginas web de las escuelas. Estudio de la documentación mediante el método comparativo. Análisis mediante una base de datos con el programa Excel. Resultados principales: Se analizan y comparan las 5 primeras escuelas y 10 de las nuevas escuelas emergentes. Conclusión principal: el trabajo permite conocer el enfoque conceptual y la metodología de las escuelas, lo que puede facilitar la elección a los profesionales y el diseño de investigaciones para aumentar la calidad de los programas (AU)


Justification: prenatal education programs are a powerful tool to increase maternal and child health. Today no one knows the most effective educational approach. Objective: To identify differences in approaching and methodology of the major schools of childbirth education. Methods: Review of the databases PubMed, Embase, Lilacs, Scielo and Cochrane since 2000. Review the websites of the schools. Study the documentation using the comparative method. Analysis using a database with Excel. Results: are analyzed and compared the 5 old one schools and 10 of emerging new one schools. Conclusions: The paper explains the conceptual approach and methodology of the schools, which can facilitate the choice for professionals and research design to increase the quality of programs (AU)

JUSTIFICATION: To improve maternal and child health is important to work the postpartum familiar reorganization in the maternal education programs. OBJECTIVES: To know the scientific evidence of the postnatal care. To look for a framework for nursing. To explore the educative strategies that midwives use in maternal education in Madrid (Spain). METHODS: Review of the evidence of prenatal and postnatal cares. Review of nursing models. Qualitative study. Probabilistic sample about 42 midwives of Primary Attention in Madrid that delivered programs of Maternal Education in 2011. Deep interviews. Interviews were coded and classified according to the method ofe based theory) through constant comparative analysis. Ethic and legal considerations. RESULTS: 14 clinic guides, 10 revisions and thousand of articles about postpartum care found. A theory of middle range found that allows to base nursing work in the familiar reorganization. 100% of midwives approached the topic and considered it very important. 36% of midwives worked with a case; 27% invited parents to a class; 14% showed the topic to pregnant woman; 11% used group dynamics; 7% through colloquium and 5% used cinema material. They did not use conceptual frameworks, neither specific evaluations, nor homework. CONCLUSION: Data allow us to know the scientific evidence, the approach that is performed in maternal education and new investigation proposals to offer care of high quality.

Justificación: para mejorar la salud maternoinfantil es importante abordar la reorganización familiar después del parto en los programas de Educación Maternal. Objetivos: conocer la evidencia científica de los cuidados postnatales. Buscar un marco teórico que fundamente el trabajo enfermero. Explorar las estrategias educativas que emplean las matronas en los programas de Educación Maternal. Metodología: revisión de la evidencia de cuidados antenatales y postnatales. Revisión de los modelos de Enfermería. Estudio cualitativo. Muestra probabilística de 42 matronas de Atención Primaria de la Comunidad de Madrid que imparten programas de Educación Maternal en el año 2011. Entrevistas en profundidad. Las transcripciones de las entrevistas fueron codificadas y clasificadas de acuerdo con el método de la teoría fundamentada mediante el análisis comparativo constante. Consideraciones éticas y legales. Resultados: catorce guías clínicas, diez revisiones y miles de artículos de cuidados postparto. Una teoría de rango medio que permite fundamentar el trabajo enfermero en la reorganización familiar. El 100% de las matronas aborda el tema y lo considera muy importante. El 36% trabaja con un caso; el 27% invita a padres; el 14% expone el tema; el 11% utiliza dinámica de grupo; el 7% realiza coloquio y el 5% emplea películas. No emplean marcos conceptuales, ni evaluaciones específicas, ni tareas para casa. Conclusión: los datos permiten conocer la evidencia científica y cómo se trabaja la reorganización familiar, con lo que se realizan propuestas de investigación para ofrecer cuidados de mayor calidad(AU)

RESUMO

Este artículo presenta un programa de salud para la atención a parejas con problemas de esterilidad, que puede desarrollarse en el hospital por matronas de una consulta específica coordinada con la consulta médica de esterilidad. El objetivo del programa es reducir el grado de ansiedad y de insatisfacción sexual generado por el proceso de búsqueda de descendencia, facilitando la información y el apoyo psicoemocional necesario.Se propone un programa de salud integrado por dos apartados: el primero: un módulo general con siete sesiones, que abarca diferentes aspectos de la salud; y el segundo, específico de sexualidad, también con siete sesiones.Al final se sugiere un cuestionario como método de evaluación del programa


This article presents a health programme directed at couples with infertility problems, which can be performed in the hospital by midwives in an specific consultation coordinated with the doctor’s infertility consultation. The objective of the programme is to reduce the levels of anxiety and sexual insatisfaction created-by the process of seeking of offspring, providing the necessary information and psycho-emotional support.An integrated health programme divided into two modules is proposed. The first is a general module with seven sessions covering different subjects topics and the second relating to sexuality specifically also with seven sessions.At the end a questionnaire is proposed as a method of evaluating the programme (AU)
